MyLife is a membership program that provides access to air medical transportation by CareJet, an air ambulance provider in Guam.

Benefits and Other Related Items
If you are a member and you suffer a serious illness or injury that is life threatening and requires specialized treatment not available on island, MyLife will fly you to the closest medical facility that can provide you that necessary treatment.
Your attending physician will need to fill out a medical necessity form. The medical necessity form states that a medical condition threatens your life, that you’re in a condition sufficiently stable for flight, and that your condition is such that air travel is not medically feasible via a commercial airline. This form must be signed by your attending physician and approved by the CareJet Medical Director. Also, your physician must source a receiving doctor and facility.

Blank medical necessity forms will be included in the membership kit that will be sent to you when you sign up. Blank forms are also provided at the GMH Intensive Care Unit and on our website here.
Your condition must be severe. Commercial airlines (like Continental) will almost always refuse to fly unstable patients for fear of worsening a delicate condition. In those instances when a commercial airline could easily -and without detriment to your health- accommodate your request, CareJet will defer this responsibility to them in order to keep our aircraft ready for life-threatening emergencies.
A receiving doctor and facility should be arranged for by the patient’s family, attending physician or health insurance provider.
We typically transport patients to Manila because of its close proximity to Guam and its abundance of world class medical facilities and specialists. However, transportation to Japan, Hong Kong or Taiwan can also be arranged for when it has been agreed by your physician and our Medical Director to be in your best interest.
CareJet can do medevacs to Honolulu. However, this is not part of the MyLife coverage.
Unfortunately, no; our aircraft must return to Guam after a transport to ensure that other members are covered. Our service only provides for one-way “bedside to bedside” transportation from the sending hospital to the receiving hospital.
Usually, a seat is reserved onboard the aircraft for a family member or friend. However, in some rare cases, additional medical personnel or equipment may be needed to ensure your safety and welfare. Understandably, the seat will be reassigned if it is in your best interest.
No, hospital costs at both ends (sending and receiving hospitals) are not included in the program.
No, none of the local health insurance companies offer this type of membership.
The primary difference between MyLife and ISOS is that we are an air ambulance operator while ISOS is an air ambulance broker. Should an ISOS member be critically injured or ill while in Guam, chances are CareJet will be the one executing the transport. Of course, MyLife members always have priority of use of our aircraft.
The Family Membership provides up to two air medical transports during each membership period. The Solo Membership provides one air medical transport per period.
If you request medical air transportation when our Guam based aircraft is not available, CareJet will deploy its Philippines based one instead. Should both aircraft be in use, we will use reasonable efforts to provide medical air transportation at the next earliest opportunity.
We will determine if another operator is needed in order to execute the mission in a timely fashion. However, medevac requests must always be submitted directly to CareJet.
No, it does not include ground ambulance services.
The air ambulance cannot carry mortal remains.
Yes, the ID will be mailed to you before your effective date.
Have the attending physician fill out a medical necessity form and call us at 472-2273.The program coordinator will then work with our Medical Director to secure the approval for transport.

Qualifications
You are only covered for air ambulance services while in Guam.
Due to aircraft weight and size constraints, members must weigh less than 300 pounds.
Yes. You are not covered for those physical or mental conditions for which you were hospitalized in the 12 months preceding your membership’s effective date.
Insurance is not required to sign up; membership covers your full transportation costs. However, one of the conditions of transport is that you must identify a receiving hospital to accept you. Having health insurance can assure coverage for those medical expenses and hospital costs.

Cost and Term
The annual membership is $375 per individual or $575 per nuclear family comprised of no more than four (4) members. Additional children may be added to the plan at a cost of $50 per child. Also, members agree to pay up to a $500 deductible per flight.
We will need to process your initial payment the day you complete your application and sign the agreement.
You can pay in full. You can also pay for the membership fee on a monthly basis. However, we only offer annual membership plans at the moment.
You can pay in full by check, credit card or debit card. You can also pay monthly by filling out an authorization form to debit your checking or savings account or charge your credit card.
We accept American Express, Visa or MasterCard.
For Solo Plans: $64.38 will be charged to your credit card when you sign up to cover the first and last months and $32.19 will be charged on the first day of the following month and every month thereafter for the remaining ten (10) months. This cost includes a 3% annual service fee only applicable to credit payments.
For Family Plans, $98.71 will be charged when first you sign up to cover the first and last months and $49.35 will be charged on the first day of the following month and every month thereafter for the remaining ten (10) months. This cost includes a 3% annual service fee only applicable to credit payments.
For Solo Plans: $62.50 will be debited when you sign up to cover the first and last months and $31.25 will be debited on the first day of the following month and every month thereafter for the remaining ten (10) months.
For Family Plans: $95.83 will be debited when you sign up to cover the first and last months and $47.92 will be debited on the first day of the following month and every month thereafter for the remaining ten (10) months.
No. It is non-transferrable and non-refundable.
Your membership is effective 30 days from the time we receive your completed application and initial payment.
Yes you can. We can fax or email you the Membership Agreement, the Membership Application, and the applicable authorization forms. After filling these out, you can return the documents to us via fax, email, or hand delivery.

Then go straight and turn right before the fenced restricted area (runway). We are on the second floor of the blue and white building at the end of the road.
Your membership will be terminated effective thirty (30) days after your payment was due.

Family Plan
The MyLife Family Plan covers one nuclear family: two (2) parents and two (2) children. At the time of enrollment, you will be asked to provide the full legal names of each prospective member. You can extend membership to additional children under the age of 18 at a yearly cost of $50 per child/teen.
Nuclear Family refers to a “family” comprised of two parents and two children. There is no marriage requirement, and the parents are not required to live at the same domicile. However, each parent must be either a birthparent or guardian to at least one of the children to qualify for this plan.
Each child must be under the age of 18 at time of enrollment. You may add additional children to the plan at a cost of $50 per child.
You do not need a full family of four (4) to avail of this plan. However, while married couples and families of three (3) are encouraged to sign up, they must pay the full cost of the Family Plan.
